Make a referral

Please note: Due to high demand, we currently have a waiting list of up to five months.

We offer support to children and young people aged 8-25 who live in Sheffield and provide care for one or more family members who have disabilities, long-term physical illnesses, mental health difficulties or who misuse drugs or alcohol.

Referral process:

Step 1: If you are working with a young person who is in a caring role, please complete a   Young Carers Assessment with them to find out what they need to make life easier. This will identify whether they need a referral to us or whether you or other professionals already working with them are the best people to offer them support around their caring. There may be some simple measures that could really help without needing to refer them to us. Please see our 'Resources to Support Young Carers' section which has lots of useful websites and resources to support you in this.

Step 2: If the assessment highlights that they would benefit from additional support through our project though, please tell them and their family about what we offer. If they decide that they would like our support, please complete our referral form with them and send it to us.

Step 3: We will email you to confirm we have received the form and let you know if we require any further information at this stage.

Step 4: Once your referral reaches the top of our waiting list*,  your form will be discussed at our team meeting. An allocated worker will then get in touch with you to arrange a joint home visit with you to meet the young person and where possible, the cared-for person, to talk through the support we can offer them.  Our support will be a time-limited intervention of one year. 

*Please note that due to high demand, we currently have a waiting list of up to three months. We will respond as soon as we can but please ensure you inform young carers and their families about our waiting time when you complete the referral with them so that they know what to expect.
