Make a referral

Please note: Apologies but, due to very high demand, we currently have a waiting list of approx. six months (unless a condition is terminal).

SYC offers one year of support to:  

  • Children and young people aged 8-25 who are living in Sheffield and caring for someone at home, usually a family member, with physical or mental illness and/or a disability. 

  • Children and young people aged 8-25 who live in Sheffield and are affected by drug and alcohol issues in their family (now or in the past).  

  • Family members, who can receive support if appropriate from our Family Project support workers.  

  • Please note: We can only support one sibling from a family at a time (the child most impacted).  

Referral process for families / self-referrals:

Step 1: Please complete our referral form with as much information as you can. If you need any help with this, please call us on 0114 258 4595 and one of our staff can arrange a time to call you and fill it in over the phone. You can complete an online referral form, or a paper version if you prefer to print and post this; both can be accessed below.  

Step 2: We will email you to confirm we have received the form and let you know if we require any further information at this stage. If you give consent to us texting you, we also send a text every month while you’re on our waiting list, to keep in touch and send you useful information while you’re waiting to join SYC.  

Step 3: Once your referral reaches the top of our waiting list, one of our workers will get in touch to arrange to phone or come and meet you, to talk through the support we can offer.  (Our support is a time-limited intervention of one year).

Referral process for professionals:

Step 1- FOR YOUNG CARERS ONLY: If you’re working with a young person who is under 18 and is in a caring role, please complete a Young Carers Assessment with them to find out what they need to make life easier. There may be some simple measures that could really help without needing to refer them to us. Please see our 'Resources to Support Young People' section which has lots of useful websites and resources to support you in this. 

Step 2: If the assessment highlights they would benefit from additional support through our project, or if they are not a young carer (i.e. affected by substance misuse in the family but not in a caring role), please tell them and their family about what we offer. If they decide that they’d like our support, please complete our referral form with them and send it to us. You can complete an online referral form, or a paper version if you prefer to print and post this; both can be accessed below. 

Step 3: We will email you to confirm we have received the form and let you know if we require any further information at this stage. If the young person/family have given consent to us texting them, we also send them a text every month while they’re on our waiting list, to keep in touch and send them useful information while they’re waiting to join SYC.  

Step 4: Once your referral reaches the top of our waiting list, one of our workers will get in touch with you to discuss the referral. They will then arrange a meeting with the young person and, where appropriate, their parent or person they care for, to talk through the support we can offer them.  Our support will be a time-limited intervention of one year.

Please note - The online form is several pages long and progress cannot be saved, so please ensure you have full details to hand before starting to fill in the form. If you are completing the referral online, we recommend that you use the word version below to gather all the information you need in advance.
